Parental Consent and Waiver for Coaching Services

This Parental Consent and Waiver (“Agreement”) is made between Coachbit, Inc. (“Coachbit”) and the undersigned Parent/Guardian, who consents to Coachbit providing coaching services to the minor child named below.

1. Agreement to services

Coachbit will provide coaching services focused on life and study skills for the child. The Parent/Guardian understands that these services are educational in nature and do not replace medical, therapeutic, or professional advice. Any necessary medical or psychological support for the child remains the responsibility of the Parent/Guardian.

2. Consent and responsibilities

The Parent/Guardian consents to Coachbit’s engagement with the child and agrees to support the child’s active participation in the coaching program. They acknowledge that coaching is not a substitute for therapy or professional services and accept responsibility for obtaining any necessary medical or psychological assistance for the child. By signing this Agreement, the Parent/Guardian releases Coachbit from any liability for direct or indirect outcomes resulting from the coaching services, including any claims for damages or injuries that may arise.

3. Confidentiality and privacy

Coachbit will maintain the confidentiality of all communications with the child, disclosing information only when required by law. The Parent/Guardian understands that Coachbit is legally obligated to report any suspected harm, abuse, or neglect affecting the child’s welfare. By signing this Agreement, the Parent/Guardian acknowledges that they have reviewed and agree to Coachbit’s Privacy Policy and Terms and Conditions.

4. Governing law

This Agreement shall be governed by and construed in accordance with the laws of the State of Delaware, United States.
